PFT Certification Renewal

Please use this form, if you have completed 4.0 or more CEU’s from our pre-approved providers such as NESTA, Spencer Institute and all other NESTA divisions, including approved providers listed here: Approved Providers.

If you need to petition CEU’s from a provider not listed in our Approved Providers list, please use the ECB Petition Submission Form first, prior to submitting your renewal.

Upon submission, you will be redirected to our secure payment page to complete the process. All major credit cards and PayPal payments are accepted. Payment in full must be received prior to reviewing your application. If payment is not made within the hour, your submission will not be processed.

NESTA certificates do not need to be submitted. If you obtained your certificates from another provider, please upload official proof of CEU completion (copy of certificate, letter of completion, etc). Allowable file extensions: pdf, jpg, png.
